Financial & Insurance Information
The Mystic Dental Group accepts most major insurances and our office staff will do the best they can to assist you in obtaining all applicable insurance benefits.
Please keep in mind that your coverage is a contract between you, your employer and the insurance company. We often advise our patients to learn about their insurance coverage and share with us any new information they receive. As a courtesy to our patients, our office will file all claims for treatment and provide your insurance company with all essential documentation and x-rays to make certain your claim is processed at the highest level of coverage.
Payment in full or insurance co-payment is collected at the time of treatment. The cost share you remit is an estimate only and the final payment from the insurance company will determine exactly how much is covered. We also participate in a convenient payment plan called "Care Credit" which allows patients to pay for treatment with interest-free monthly payments. Checks and all major credit cards are gladly accepted.
When you call to make your appointment, please feel free to ask any questions you may have.
What You Need To Know Before Your First Appointment
Please arrive for your first visit 15 minutes early and be prepared to complete all insurance and health information forms. We will ask you to fill out several forms that will help us to get acquainted with you and your dental needs.
At the time of your appointment, please come prepared with the following information:
- Any radiographs from your previous dentist
- A list of medications you are currently taking
- Dental insurance card(s)
It is important that a parent or guardian be present with any patient under the age of 18.
