What You Need to Know Before Your First Appointment
Please arrive for your first visit 15 minutes early and be prepared to complete and/or review all new patient forms with our dental coordinator. These forms are required and assist us in getting acquainted with you and your dental needs.
At the time of your appointment, please come prepared with the following information:
- Any radiographs from your previous dentist
- A list of medications you are currently taking
- Dental insurance card(s)
It is important that a parent or guardian be present with any patient under the age of 18.
For your convenience, some of the following forms are available to download and fill out prior to your appointment. This will save about 15 minutes of your time in our office.
